Patient Forms
The first step to starting treatment with me is for us to have a brief (free) phone conversation to discuss any questions you may have and for me to better understand if the type of treatment you need is something that I offer. If we decide to schedule an intake, you will be directed to my patient portal to fill out forms including consenting to my practice policies.
Medication Refills
Requests for medication refills should be made at the time of your appointment. Patients are responsible for ensuring that they have adequate refills until the next appointment. If you are to find that you need refills in between appointments, I should be contacted directly via Luminello patient portal or phone call.
It is important to contact me directly rather than relying on automatic pharmacy refill requests as these do not always reflect our most recent changes.
